Clinical Review on Surgical Jaundiced Patients



Abstract
During the period of 15 years from October, 1957 to December, 1971, 292 cases of surgical jaundice were treated at this surgical department in Busan National University Hospital.
These surgical jaundiced patients had frequently more serious symptoms and more higher death rate in compared with patients without jaundicefollowing surgical procedures. Authers reviewed for these 292 cases to clarify the cause of high mortality rate and summarized as follows:
1) The over all incidence was 4.68%. The most predominant incidence of 292 surgical jaundiced patients were 144 cases of common bile duct stone, 48. 7%, next commondiseases of them were 60 cases of liver abscess, 18. 4%, third common diseases of them were 20 cases of hepatoma, 6. 9%, and the least incidence of them were 4 cases of choledocal cyst, 1. 3%.
2) The predominant age group was from 41 year old to 60 year old and the number of these patients was 209 cases, 71. 7% and the age distribution showed from 3 months old to 74 years old.
3) The ratio of male and female was 1.4 : 1.
4) The most common operative procedures were Choledochostomy with T-tube insertion and their numbers were 141 cases, 48. 3%, next common groups were i & d of Liver abscess, their numbers were 53 cases, 18.200, and third common group were hepatectomy, their numbers were 20 cases, 6. 5%,
5) The over all postoprative complications in this series was 39 cases, 12. 7%. The most common postoperative complication bile peritonitis and their numbers were 9 cases, 3. 1%, next common group was cholangitis,, their number were 8 cases, 2. 7%, and third common group was hemorrhage, their number were 5 cases, 1. 8%.
6) The over all mortality rate in this series was 6.16%, 18 cases.
7) The causes of death after operation for the jaundiced patients during admission were 6 cases of bile peritonitis, 33. 3%, 5 cases of cholangitis, 27. 8%, and each 2 cases of hemorrhage and suspected hepatorenal syndrome, 11. 1%, respectively.
